¹ Rebate is applicable for Business DSL 1.5, 3.0, 6.0, 8.0, 10.0 or 15.0.

² Only Business DSL Plus 384, 768 Kbps, and 1.1, 1.5 Mbps are eligible for the router rebate. Customers will be charged sales tax based on the value of the router, even if cost of router is waived or rebated.

Rules and Regulations:
To qualify, Customer must order Covad service directly from Covad and have Covad service installed directly by Covad. Customers receiving Covad service through any other Covad partner or sales channel do not qualify for this offer. Orders must be placed between November 1 - 30, 2009 directly from Covad. If Customer requests a downgraded speed after installation and after a request for rebate has been submitted or processed, Covad will assess all applicable termination charges and Customer will be required to forfeit or return the rebate. Customer may apply for rebate as soon as order is placed. Rebate checks will not be processed unless the following apply: Customer must (1) complete the 30-day satisfaction guarantee period after installation, (2) pay its first invoice in full, and (3) complete online rebate submission by December 31, 2009. Covad will not pay rebates on any service canceled during the 30-day satisfaction guarantee period or for any account that is not current in all payments owed to Covad. Checks will be issued to the Customer name and company name on the order and mailed to the address where the Covad service was installed. Information provided on the rebate form will not be transferred or sold by Covad. Limit one rebate per Customer per service ordered. Duplicate requests will not be honored. All submissions will become the property of Covad Communications Company. Offer valid for U.S. residents only. Offer good only in those states in the U.S.A. where it is not prohibited, taxed or restricted by law. Please allow 6 to 8 weeks after Customer's 30-day satisfaction guarantee period is completed for delivery of the rebate check. Rebate checks not cashed within ninety (90) days will be cancelled. Covad reserves the rights to reject any rebate form that it deems, at its sole and absolute discretion, to be fraudulent, duplicative, incomplete or not to be in the proper form.
